Certified diver over 60 years old to take part in a dive tour
People with major diseases and injuries in the past, history of surgery

Please check the questionnaire about illness, and the one that applies is "Yes", but please fill in "No" for nothing at all.

Please fill customer information in the entry field on the second page.
Please write your doctor 's signature at your nearest hospital to see if it is suitable for diving.*
*It is necessary only for those who have "Yes" in the questionnaire, or those over the age of 60.
